- Please forward three copies of your entity’s Annual Report or Audited Financial Statements(Applicable only for Non-Listed Companies with Export Revenue Exceeding USD 50 Million-Refer to Note 3) for the year ended 31st December 2025 or 31st March 2026, as applicable, on or before the TAGS Awards application deadline of 31st August 2026.

Note 03:
Criteria applicable for Non-Listed Companies with Export Revenue Exceeding USD 50 Million for Special Awards
- Non-listed companies with export revenue exceeding USD 50 million are eligible only for the Corporate Governance Disclosure Award and Sustainability Reporting Award.
- Applicants should submit reports containing relevant information for the respective award. Such reports should be available for public review on the company’s website, social media, etc.
- A certification from the Statutory Auditor on financial reporting and audit opinion is required.

1. Originality of the Annual Report
We confirm that the Annual Report submitted for the TAGS Awards is the same report issued to shareholders and, where applicable, submitted to the Colombo Stock Exchange and relevant regulatory authorities. No alterations have been made solely for the purpose of this application.

We certify that the information provided in this application and the submitted Annual Report is true, complete and accurate to the best of our knowledge.
2. Errata, Corrections and Restatements
Please indicate whether any errata, correction, revision or restatement has been issued in relation to the submitted Annual Report.
2.1 Undertaking for Errata
We undertake to promptly notify the TAGS Awards Secretariat of any errata, correction, revision or restatement issued after the submission of this application.
3. CSE Watchlist – Transfer Into/out of a Watchlist
3.1 Transfer in
Please indicate whether the Company was transferred into a Colombo Stock Exchange (CSE) Watchlist category during the reporting period or after the reporting date but before the submission of this application.
3.2 Transfer Out
Please indicate whether the Company was transferred out of a Colombo Stock Exchange (CSE) Watchlist category during the reporting period or after the reporting date but before the submission of this application.
4. Notification of Subsequent Watchlist Transfers
We undertake to promptly notify the TAGS Awards Secretariat of any transfer into or out of a CSE Watchlist category occurring after the submission of this application and prior to the conclusion of the awards process.
5. Material Events and Regulatory Matters
We undertake to notify the TAGS Awards Secretariat of any material event, regulatory action, investigation, enforcement action or other significant development occurring prior to the conclusion of the awards process that may have a material impact on the contents of the submitted Annual Report.
6. Availability of Digital Annual Report
We confirm that the digital version of the submitted Annual Report is available at the time of issuing the report and will remain publicly accessible throughout the evaluation period.
7. Availability of Related Digital Disclosures
We confirm that all hyperlinks, QR codes, microsites, sustainability reporting platforms and other digital resources referred to in the submitted Annual Report as available at the time of issuing the Annual Report and will remain accessible throughout the evaluation period.
8. Evaluator Clarifications
We agree to provide additional information or clarifications requested by the TAGS Awards Secretariat, evaluators or judges within the stipulated timelines.
9. TAGS Compliant Badge – Acceptance of Terms and Conditions (Annexure II)
9.1 We confirm that we have read, understood and agree to comply with the TAGS Compliant Badge Terms and Conditions (Refer to Annexure II) issued by CA Sri Lanka in relation to the use of such badge for marketing & promotional purposes, .
9.2 We acknowledge that authorization to use the TAGS Compliant Badge, where applicable, is subject to compliance with those Terms and Conditions and may be withdrawn in accordance with the applicable guidelines.
10. Consent for Evaluation and Promotional Use
We consent to CA Sri Lanka using references to the submitted Annual Report, including report covers, extracts, rankings, scores, recognitions and award achievements, for evaluation, benchmarking, educational, promotional and awards-related purposes.
11. Acceptance of TAGS Awards eligibility Criteria and Decisions
We confirm that we have read and agree to comply with the TAGS Awards Eligibility Criteria. We agree to abide by the decisions of the TAGS Awards Organizers, which shall be final.

Overall Terms and Conditions
- Entries for the awards will be accepted or rejected at the discretion of the Institute of Chartered Accountants of Sri Lanka.
- The Institute of Chartered Accountants of Sri Lanka reserves the right to re-categorise any category applied for, if necessary.
- The decision of the Panel of Judges will be final and binding on all applicants.
- Winners, including certificate recipients, of the TAGS AWARDS are permitted to use only the “TAGS” logo for their communications, including newspaper and digital advertising, and cannot use the “CA Sri Lanka” logo for any communication purposes in relation to the TAGS AWARDS.

I hereby certify that the information provided in this application and all supporting documents is true, complete and accurate to the best of my knowledge, and I am duly authorized to submit this application on behalf of the organization.
